NAB Forms - use appVar to toggle DisableFields(*all)

Some users can add and edit data in a form, but maybe others can only view the data. If we could toggle edit/read-only at a form level then we could use one form for both types of users.

Ideally, this would also work when using the form as the edit widget for the Edit Grid.
