In the API docs for vvOut_toJSON example #3 it shows how you don't have to use an externally defined object, but it also says it works "best" for single column grids or drop downs. Does it really mean you can ONLY have one field?

Yes, if you don't specify an externally defined object then you can only have one field. One alternative is to build your own JSON output as a string and use vvOut_data to return it to the browser. RPG's string handling makes this pretty easy to accomplish along with the Valence Test RPG program app.
